**IMPORTANT TO NOTE – HOUSING CHANGE**
Our program has been moved to the Pegram Residence Hall located at 1340 W Markham Ave, Durham, NC 27705. This is where students will now check into the program.

Forms Requiring Parent/Guardian Signature

The following forms are issued by Duke University’s Minors on Campus Department and are required to be signed and submitted by a parent or guardian on behalf of each student participating this summer. Please click the link below to access the forms, and electronically sign and submit them NO LATER THAN July 13, 2023.
